Cost
Due to the complexity of care and time required to review each individual request, we charge a fee of $500.00 for this service. Payment is due at the time of the request and all major credit cards are accepted (Visa, MasterCard, American Express and Discover.) We do not accept payment by check.
Insurance
We do not submit claims or bills to insurance for this image review service. If you anticipate a trip to Vail for treatment or surgery and would like to learn more about your benefits and coverage please call your insurance company directly and provide them with the clinic’s TAX ID #84-1415470
To request a film review, please follow the steps below.
We do ask for all the below associated imaging reports for any and all imaging sent (within 1 year):
Please send copies of your imaging studies and not original films as they will not be returned (sorry, no exceptions).
Send your images and records to our office and to the attention of Dr. Lee’s Team. Delivery options are listed below:
After all materials are received, a member of the team will contact you to confirm your image review is received successfully and will charge your card for payment in anticipation of the review. Lastly, someone on Dr. Lee's team will reach out to schedule this image review phone call.
Someone on our team will begin the image review phone call with a general medical history. They will call the phone number you provide in the packet. Dr. Lee will then review your image findings and his treatment recommendations with you.
Please allow 7-14 business days for image review processing and phone call follow-up.
